Membership supports the time, access, continuity, and clinical involvement that are difficult to provide in a traditional high-volume practice. You may join as an individual, add household members, or choose a family membership.

Founding Membership

Clear options for individuals and families.

Individual membership begins at $3,800 annually, with family and dependent membership options available. Membership services are provided outside the insurance billing model, so included visits do not generate insurance copays.

Memberships are billed annually or semiannually at the stated annual rate. Quarterly billing is available at a modestly higher total.

Individual Membership

For one adult member.

Membership does not require enrolling a spouse or family. Additional household members may be added at the age-based rates below.

Household option

Family Membership

$8,300annual membership

Two adults and up to three dependent children.

Dependent children may be enrolled through age 26. The family membership serves as a household cap for families that meet this structure.

Designed for households that want one physician relationship spanning parents, children, and the health decisions that affect the family together.

Two-Year Rate LockFounding rates are guaranteed for each member’s first two years.
Ongoing Founder BenefitAfter two years, Founding Circle members receive 10% off the then-current standard membership rate while continuously enrolled.
